FLORIDA HOSPITAL Volusia Home Care Services is located in the suite 209 of the Summit Building located on FLORIDA HOSPITAL Fish Memorial's campus and is licensed to serve Volusia county.

We provide our patients with registered nurses, home health aides, physical therapists, occupational therapists, medical social workers and chaplains.

You must be homebound, under the care of a physician and require a skill (Nursing, PT or OT) in order to qualify. Under the supervision of your physician, we develop an individualized program to provide you with the care you need.

If you or someone you know meet the above criteria and want to receive quality skilled care in the comfort of your home, we are the home care agency for you.

A referral can be made by:

• A phone call from your physician's office
• The discharge planner from the hospital
• Case manager from a skilled nursing facility
• You, a friend or a family member can call us to see if you qualify

• At your request and with no charge, the emotional
  and spiritual support of a chaplain.
