Going Home
Your physician has the responsibility of deciding when you are well enough to leave the hospital and must complete a “discharge order” for you to leave. Our goal is for each patient to leave the hospital with all the information they may need for the best health possible, including information about medications, activity level as advised by the physician, diet, outpatient therapy and follow-up visits. Our plan is to review this information with you or your caregiver, and to verify your understanding, in order for you to have the best possible outcome after your hospital visit.

As you prepare to leave, please check all closets and drawers for your belongings. Upon discharge, a hospital employee or volunteer will escort you to the exit so that you may proceed to your destination. If needed, your nurse or the information desk staff (located in the main lobby) will help you call for a ride should you need transportation assistance.


Case Management/Social Workers
We recommend you discuss with your physician his/her recommendations for care once you leave the hospital. To prepare you for discharge and continued care that may be needed following your hospital stay, our Case Managers and Social Workers are available to assist you. Our team is available to communicate with your doctor as needed and can assist you and your family in meeting your health care needs after your hospitalization.

You may wish to discuss any of the following with the Social worker:
• An assessment of your home care situation
• Information about community resources
• Counseling regarding your medical and home care situation
• Guidance about home care services
• Skilled nursing facility options
• Resources for durable medical equipment

Home Health Care
FLORIDA HOSPITAL Volusia Home Case Services, a service of FLORIDA HOSPITAL Fish Memorial, offers care for those who need additional nursing care when they go home. Services include skilled nursing, intravenous therapy, physical therapy, speech therapy, occupational therapy, medical social services and home-health aids to assist in bathing and dressing as well as other daily activities. A home care coordinator is available to help your assess your needs and verify insurance coverage for home care.

When you Arrive Home
After you get home, if you should have any problems or questions related to your health, your medications or your follow-up plain including results of any tests performed during your hospital stay, please call your primary care physician.

Be sure to call your doctor to make your follow-up appointment and fill any new prescriptions at the pharmacy.
